An implant had been placed all the way to the tip and the tip had been raised with ear cartilage, but over time the tip deformed, the alar base spread, and the columella protruded downward.
The silicone implant had been inserted too shallowly and was noticeable from the outside.

Tip plasty was performed with septal cartilage; the spread alar base was narrowed without any incision in the alar base, and the nostrils also became slender.
The implant placed in the dorsum is not visible at all from the outside, and the shape has become natural.
