Summary Lip filler (horizontal widening) is a treatment that, rather than making the lips thicker, adjusts their horizontal length and border line to create natural mouth proportions and a natural smile line. It is planned around the proportions of the area around the mouth and is performed at RESUM Plastic Surgery in Gangnam (Apgujeong), Seoul. Medical Review Medical Review: Won-Suk Kook,Jin-Young Kim

Why Lisoom

Overview Refining the Horizontal Line at the Corners of the Mouth
for Longer-Looking Lips

Unlike treatments that increase the volume of the lips themselves, lip filler (horizontal widening) uses cross-linked hyaluronic acid (HA) to extend the line along the horizontal length at the corners of the mouth and the lip border (vermilion border), adjusting the proportions of the mouth.
Without plumping volume above and below, it lengthens the lateral line to create a natural smile line, and the product used and the injection sites differ from those of standard lip filler.
The superior and inferior labial arteries run near the corners of the mouth, so depth control is important and a cannula is given priority; because bruising, nodules, and other responses vary by individual, we proceed conservatively.

Mouth Corner Line SUTURE guide image

SutureMouth Corner Line

The line is extended subtly in the horizontal direction from the end of the mouth corner along the vermilion border. Because many branches of the superior and inferior labial arteries are distributed around the corners of the mouth, small amounts are injected slowly at a safe depth. If the corners of the mouth angle downward, we also discuss combining the treatment with Botox.

Lip Border HYBRID guide image

HybridLip Border

The lip border is thinly reinforced using the retrograde linear technique to make the line more defined. For the Cupid's bow of the upper lip, controlling the amount is important so that its shape is not lost. The horizontal line is emphasized while taking care that the lips do not look overly projected upward.

Proportion Adjustment PARTIAL guide image

PartialProportion Adjustment

The extent of horizontal widening is set by assessing the horizontal length of the mouth relative to facial width, the ratio of the upper lip to the lower lip, and the length of the philtrum together. If the reason the mouth looks small lies in the philtrum length or chin proportions, Botox or another treatment may be more suitable, and we tell you so candidly at the consultation.

Combined with Volume INCISION guide image

IncisionCombined with Volume

If you would like both horizontal widening and added volume in the lips themselves, products of different viscosities are used separately for each area, within 1ml for 1 session. Rather than filling for both at once, we recommend establishing the horizontal line first and adding volume in stages.

OUR PRINCIPLE

Core Values
The Principles RESUM Keeps

Horizontal widening is not a treatment that plumps the lips; it is a treatment that balances proportions. Rather than extending a lot at once, starting with less and refining gradually looks more natural.

Right Ratio

Right Ratio

Proportions First

The extent of horizontal widening is set by assessing facial width, philtrum length, and the ratio of the upper lip to the lower lip together. Rather than simply extending the corners of the mouth, we look for a natural width within the overall facial proportions. The decision is made at the consultation while looking at photos of the patient's face together.

Safe Depth

Safe Depth

Safe Depth

Because many branches of the superior and inferior labial arteries are distributed around the corners of the mouth, control of safe depth and use of a cannula are important. Small amounts are injected slowly while the pressure is monitored. We check in advance that a dissolving enzyme (hyaluronidase) can be used immediately in an emergency.

Honest Limit

Honest Limit

Honest Limitations

If the reason the mouth looks small lies not in its horizontal length but in the philtrum length or chin proportions, horizontal widening alone is unlikely to bring a significant change. In such cases, we candidly discuss another treatment, a combination with Botox, or a decision not to recommend treatment at all.

How HA Fillers Work

How HA Fillers Work

Lip filler (horizontal widening) works by injecting a gel made by cross-linking hyaluronic acid, a component naturally present in the skin, with BDDE or a similar agent. The cross-linked structure makes the gel more stable so that it does not break down easily, helping the horizontal line to be maintained for a certain period. Because the lips move a great deal with facial expression, the effect tends to last a shorter time than in other areas. Cross-linked HA gel injected along the horizontal line at the corners of the mouth. BDDE cross-linking slows breakdown, supporting longevity. A soft horizontal line through water affinity. Partly adjustable with hyaluronidase if needed.

Planning Tailored to Each Area (Viscoelasticity and Injection Layer)

Planning Tailored to Each Area (Viscoelasticity and Injection Layer)

Because horizontal widening is planned to lengthen the lateral line along the lip border (vermilion border), the product and injection sites differ from those of standard lip filler. Without plumping volume above and below, the horizontal length at the corners of the mouth is adjusted to fit the proportions, with facial width and the ratio of the upper and lower lips assessed together. We review the distribution of the superior and inferior labial arteries to control depth and give priority to a cannula. Distributed laterally along the vermilion border. Horizontal proportions adjusted instead of volume above and below. Facial width and lip proportions assessed together. Superior and inferior labial arteries considered, cannula first.

Precautions
Aftercare & Precautions

N · 01 Horizontal widening is not a treatment that plumps the lips; it is a treatment that balances the horizontal proportions of the mouth.
N · 02 Rather than extending a lot at once, we start with less and refine gradually to reduce an upward-projected look.
N · 03 Because many branches of the superior and inferior labial arteries are distributed around the corners of the mouth, small amounts are injected slowly at a safe depth.
N · 04 The lips move a great deal with facial expression, so swelling and tenderness may be more noticeable than in other areas for the first 1-2 days after treatment.
N · 05 Controlling the amount is important so that the Cupid's bow of the upper lip keeps its shape, and the filler is distributed so that nodules do not form.
N · 06 If you have a history of herpes simplex (cold sores), we decide on preventive medication in advance; treatment is not performed during pregnancy.
N · 07 Results may vary depending on individual constitution and treatment condition, and a consultation with a board-certified specialist is required before treatment.

Doctor's Note

Horizontal widening is not a treatment that plumps the lips; it is a treatment that balances proportions. We do not look only at the corners of the mouth but assess facial width, philtrum length, and chin proportions together. We keep to academically validated safe depths and proceed conservatively, within 1ml for 1 session.

FREQUENTLY ASKED

FAQ
Frequently Asked Questions

We have compiled the questions patients ask most often before a consultation.

Unlike treatments that increase the volume of the lips themselves, this treatment adjusts the proportions of the mouth by extending the line along the horizontal length at the corners of the mouth and the vermilion border. It is often recommended when the mouth looks small or the corners of the mouth appear to droop. Without plumping volume above and below, it lengthens the lateral line to create a natural smile line.
Standard lip filler adds volume to the lips themselves, whereas horizontal widening adjusts the horizontal line and proportions at the corners of the mouth. The product viscosity and injection sites differ. If you would like both treatments together, we use separate products for each area.
Hyaluronic acid settles in place immediately, so a change in contour is visible right after treatment. Swelling may occur for the first 1-2 days, so the contour stabilizes at around 1 week. We recommend assessing the final contour after the swelling has subsided.
You can return to daily activities the same day. Swelling and tenderness settle within a few days, and the final, stable contour takes shape at around 1 week. Swelling can be more noticeable on the lips than in other areas, so it is advisable to keep your schedule flexible for the first 2-3 days.
Temporary swelling, bruising, and tenderness may occur and settle within a few days. Nodules, asymmetry, and hypersensitivity reactions have been reported in rare cases. Vascular complications are reported very rarely; to lower the risk, safe injection depth, use of a cannula, and having a dissolving enzyme on hand are important.
Pricing depends on the product used and the amount injected. At the consultation we assess your contours, decide on the product and amount, and then provide a quote. Horizontal widening alone and treatment combined with volume are quoted differently.
