Summary Tear trough filler is a treatment that fills the tear trough and under-eye shadow at a safe depth to brighten the area; when the cause is fat protrusion or pigmentation rather than hollowing, filler may not be suitable. At RESUM Plastic Surgery in Gangnam (Apgujeong), Seoul, treatment proceeds after the cause has been assessed. Medical Review Medical Review: Won-Suk Kook,Jin-Young Kim

Why Lisoom

Overview The Area That Demands the Most Delicacy,
Treated Carefully with the Right Amount

Tear trough filler is a treatment in which cross-linked hyaluronic acid (HA) is injected at a safe depth to correct the under-eye shadow and hollowing seen along with the tear trough.
Under-eye shadow is a mix of hollowing, pigmentation, skin thickness, the orbicularis oculi muscle, and fat protrusion, so not all dark circles can be corrected with filler, and another method may be suitable for pigmented dark circles.
Because the skin is thin, a small amount of a soft product with a low G' is injected with a cannula into the deep layer above the periosteum at the orbital rim; bruising, the Tyndall effect, and other responses vary by individual, so we proceed conservatively.

Safe Depth SUTURE guide image

SutureSafe Depth

The basic depth is the deep layer above the periosteum (preperiosteal plane) at the orbital rim. The superficial layer close to the dermis is avoided because of the possibility of the Tyndall effect (a bluish shadow), and we do not inject directly into the orbicularis oculi muscle. Because the skin here is thin, control of depth determines the result.

Cannula Use HYBRID guide image

HybridCannula Use

The angular artery runs close to the under-eye area, so we give priority to a cannula to lower the risk of vascular injury. We inject small amounts slowly and monitor the pressure. To prepare for possible vascular complications, we check in advance that a dissolving enzyme (hyaluronidase) is on hand and that emergency procedures are in place.

Soft Product PARTIAL guide image

PartialSoft Product

For the under-eye area, we use soft products with a low G' value and a high absorption rate. Injecting a highly cohesive product close to the surface tends instead to make the contour stand out or to form nodules. The choice is made at the consultation based on suitability for the area.

Volume Control INCISION guide image

IncisionVolume Control

The amount injected in 1 session is kept conservative, within 0.5-1ml for both sides combined. Our basic approach is staged: we start with less and add more after 4 weeks. Overfilling causes a puffy contour and nodules, and removing filler once it has been placed is the more burdensome process.

The Principles RESUM Keeps

The under-eye skin is thin, making this the area where control of depth and amount matters most. We first identify the cause of the shadow and proceed only when filler is suitable.

Right Cause

Right Cause

Cause Assessment

The causes of under-eye shadow are a mix of several factors, including hollowing, pigmentation, skin thickness, the thickness of the orbicularis oculi muscle, and fat protrusion, so not every shadow can be corrected with filler. At the consultation we assess the cause of the shadow and then suggest a suitable method, whether filler or another procedure (such as laser, a PN booster, or fat repositioning surgery).

Safe Depth

Safe Depth

A small amount is injected with a cannula into the deep layer above the periosteum at the orbital rim. The superficial layer close to the dermis is avoided because it causes the Tyndall effect, and the inside of the orbicularis oculi muscle because it causes bruising and swelling. We review the course of the angular artery and control the safe depth.

How HA Fillers Work

How HA Fillers Work

Tear trough filler works by injecting a gel made by cross-linking hyaluronic acid, a component naturally present in the skin, with BDDE or a similar agent. The cross-linked structure makes the gel more stable so that it does not break down easily, helping volume to be maintained for a certain period. The injected HA attracts water, gently filling the hollow, shadowed area. Cross-linked HA gel injected at a safe depth under the eyes · BDDE cross-linking slows breakdown, supporting longevity · Shadow gently filled through water affinity · Partly adjustable with hyaluronidase if needed

NOTICE

Precautions
Aftercare & Precautions

N · 01 The under-eye area has the thinnest skin, so the amount is kept conservative, within 0.5-1ml for both sides combined.
N · 02 If the filler sits in the superficial layer close to the dermis, the Tyndall effect, in which a bluish shadow shows through, can become noticeable.
N · 03 The orbicularis oculi muscle is thin and rich in blood vessels, so injecting into it makes edema and bruising more pronounced than in other areas.
N · 04 The angular artery runs close by, and vascular complications have been reported very rarely, so we give priority to a cannula.
N · 05 Pigmented dark circles and shadows caused by fat protrusion are not addressed by filler, so we may suggest another method.
N · 06 Treatment is not performed during pregnancy or on areas with active skin infection or severe inflammation.
N · 07 Results may vary depending on individual constitution and treatment condition, and a consultation with a board-certified specialist is required before treatment.

FAQ
Frequently Asked Questions

We have compiled the questions patients ask most often before a consultation.

Tear trough filler is a treatment in which cross-linked hyaluronic acid is injected above the periosteum at the orbital rim to correct the under-eye shadow seen along with the tear trough. Filler is not the answer for every under-eye shadow; at the consultation we first assess what is causing the shadow and proceed only when filler is suitable.
We use soft products with a low G' value and a high absorption rate. Using a highly cohesive product under the eyes tends instead to make the contour stand out or to form nodules. The choice is made at the consultation based on suitability for the area.
Hyaluronic acid settles in place immediately, so a change in contour is visible right after treatment. Swelling and bruising may occur for the first 1-2 days, so the contour stabilizes after 1-2 weeks. We recommend assessing the final contour after the swelling and bruising have subsided.
You can return to daily activities the same day. Swelling and bruising settle within a few days and, depending on the area, may last about 1 week. Because the under-eye skin is thin, bruising can be more noticeable, so it is advisable to keep your schedule light for the first 3-5 days.
Temporary swelling, bruising, and tenderness may occur and settle within a few days. The Tyndall effect (a bluish shadow), nodules, asymmetry, and hypersensitivity reactions have been reported in rare cases. Vascular complications are reported very rarely; to lower the risk, safe injection depth, use of a cannula, and having a dissolving enzyme on hand are important.
Pricing depends on the product used and the amount injected. At the consultation we assess the condition of your under-eye area, decide the product and amount, and then provide a quote. On average, the amount used falls within the range of 0.5-1ml.
