Mentalis Resuspension: Case II - Photo 1

Recently, among contouring surgeries, the procedure that resects a wide chin bone or a long, protruding chin

to reduce the length and width of the chin and make it slimmer,

“T-osteotomy (V-line) chin surgery,” has become widely known as something of a trend, and many people do undergo it.

However, after osteotomy-type chin contouring surgery,

a problem often arises in which the chin muscle (mentalis muscle) sags downward.

The same occurs when a receding chin has been corrected with a silicone implant

and the silicone implant is later removed.

This problem is diagnosed as “Witch’s chin” or “chin muscle ptosis.”

The mouth can be closed only with constant effort, the chin muscle tightens and the chin looks longer,

and straining the lips produces pebbled chin dimpling, neck wrinkles, and the like,

causing both discomfort and cosmetic dissatisfaction.

After chin contouring surgery, some people notice this “chin muscle ptosis” but

do not know that it is a side effect that can be improved with “mentalis resuspension” surgery,

and surprisingly many of them struggle with it as a result.

Now, let us look more closely through a “mentalis resuspension” case.

◎ Diagnosis: “Witch’s chin” or “chin muscle ptosis”
◎ History: double-jaw surgery at a university hospital in Seoul 20 years ago ▶▶ chin contouring surgery in Gangnam 5 years ago ▶▶ mentalis resuspension in Gangnam 4 years ago ▶▶
◎ Main symptoms: the mouth does not close, leading to a habit of straining the lips/the lower lip rolls inward/the chin muscle sags downward and the chin looks longer/a double-chin crease appears under the chin.

This shows the mouth closed.
Before surgery, the lips were strained and pursed,
but after surgery they are relaxed and naturally spread, without tension.
In the chin muscle as well, the pebbled dimpling is pronounced in the tense preoperative state,
but after surgery it has naturally disappeared and the chin looks at ease.

With the mouth naturally open: muscle tension is low, and the mouth opens naturally.

Before surgery, the lower lip is rolled inward, and the chin muscle sags downward, showing a double crease under the chin.
After mentalis resuspension surgery, the double-chin crease under the chin has disappeared andthe chin has naturally become shorter. At the same time, the lower lip that had been rolled inward has come forward and looks natural, and the chin is taut.

Even with the lips naturally parted, the sagging chin is now taut,
the double chin is gone, and the lower lip, which had been excessively tucked in, also parts naturally.

In chin contouring surgery, the chin bone is separated from the muscle,
the bone is resected or an implant is placed over the bone, and the tissues are then sutured back.

In this process, the muscle often fails to reattach to the bone in its proper position
and sags downward.

As seen in this case, the muscle may sag downward and lengthen the chin,
the lower lip may drop and roll inward,
or a double chin may develop.

These side effects are called Witch’s Chin,
and by lifting the chin muscle and fixing it to the bone below the gums so that the muscle
returns to its proper position, a correction known as “mentalis resuspension,”
the natural line of the chin can be restored.
