Tip Cartilage Repositioning and Nasal Implant Removal: Case #5

Tip Cartilage Repositioning and Nasal Implant Removal: Case #5
(Removal of nasal implant and tip cartilage reposition)

The patient in this case had, 12 years earlier at another clinic,
undergone silicone implantation + septal strut placement + dorsal hump removal through an open approach.
A problem later developed at the columella inside the nose, and a re-correction was performed through a closed approach 8 years ago.
Even after the re-correction, however, something was protruding on the inner side of the columella,
and the patient visited our clinic because of cartilage showing through the skin, a nasal shape different from before, a feeling of pressure, and a stabbing sensation.
Examination showed contracture, which had caused the nasal tip to turn upward.
The silicone implant was deviated, with its outline visible from the outside,
and the nasal tip was shifted to one side with a tilted columella.
The nostrils were asymmetric, with alar notching, in which the ala is pulled upward.

After removing the silicone, the capsule, and the columellar strut,
“tip cartilage repositioning” was performed so that the nose could return to its original appearance.

4 months after tip cartilage repositioning and nasal implant removal
The previously crooked nose has been straightened.
With the silicone removed, the outline that had been showing through is gone,
and the nose has returned to its original, natural appearance.
The foreign body sensation from the protrusion on the inner side of the columella has also been resolved.

The tilted nasal tip and columella are now straight.
The nostrils, which had been uneven and asymmetric, are now symmetric.

The nasal tip that had been lifted by contracture came down considerably after surgery,
and the nasal bridge is longer overall and extends in a clean line.

The patient was satisfied that the various problems with the nose were improved at once
and that the nasal tip and overall shape of the nose became soft and natural.
