Tip Cartilage Repositioning and Septal Extension Graft: Case #4
(Removal of nasal implant and tip cartilage reposition)
The patient in case 4 had undergone tip plasty twice, 3 years ago and 1 year ago,
and after a final septal extension graft 2 months ago,
the nasal tip became excessively high and pointed, and deviated.
Examination showed high tension from the septal extension graft at the nasal tip,
and the nasal tip was deviated to the patient's left.

Surgery was performed through a closed approach: the septal extension graft was removed from inside the nose,
and the deviated tip cartilage was repositioned to its original position and straightened.

After surgery, the dorsum and nasal tip that had been deviated to the left are straight.
The tension caused by the septal extension graft has been released, and the nose looks natural.

From below as well, the previously tilted columella can be seen to be straight,
and the asymmetry of the nostrils has been completely corrected.

From the side as well, the nasal tip, which had looked as if it were about to protrude,
can be seen to have returned to a gentle, soft shape.
